Presidential Newsletter - 3/22/10 Print E-mail

Dear Members and Friends of TOMA,

Well, we hope that is the last blast of winter. We had osteopathic friends in from Philadelphia, and they brought North Eastern weather with them. Pennsylvania Osteopathic Medical Association board of trustees member Dick Purse and his wife, APOMA past-president Leanne Purse were here for an educational seminar for osteopathic radiologists. AAOA past-presidents Rita Baker, Shirley Bayles and Linda Cole took Leanne on a shopping and cuisine tour of the metroplex. Following a marvelous dinner with the Purses on Saturday evening, we awakened to four inches of snow in Dallas. Next time, we will send Dallas weather home with them.

      By the time you read these words, the elephant in the room will probably have broken out of the confines of back-room politics to ‘fix’ the system on which we depend for our health care, and to pay our family bills.  Speaker Pelosi has scheduled a vote for Sunday that will pass the hotly contested health financing reform package. There have apparently been enough negotiated deals for the controlling votes to vote with the president. The Nebraska and Louisiana bribes are gone, but there are hundreds of millions of dollars in payoffs that will eventually become public knowledge. It is estimated that insurance company and other interested lobbyists have ‘invested’ over $1 million per congressman to influence the outcome of this mandate from the president. These businesses obviously believe that the federal subsidies will be beneficial to their profit structure. The president has won this round, and taxpayers will evaluate the progress. We will see how it plays out in November.

Dear Members and Friends of TOMA,

            This afternoon, I threw caution to the wind and followed the advice that I give to patients. You know the one, to improve your cardiovascular health, to stimulate your immune system, and to help your waistline, exercise 30 minutes every day. You don’t have to sweat, just get your pulse rate up. I was sitting in front of a warm and cheery fire, and was watching the light rain bring needed ingredients for our spring flowers. Our dog began begging to go for a walk. Rex is a 15 year-old cocker spaniel who is very healthy because he spent the first 13 years of his life running the hills around Grand Lake. He now lives for his walks, and he makes it known when he is ready. I remembered my advice to others, if it is cold put on a coat, and if it is wet, put on a hat. So we went out to a nearby field for our bonding experience. It rained the entire 45 minutes that we were out, and he had to be corralled to come back. Eddie Chiles was an oilman and was a former owner of the Texas Rangers. He famously said ‘If you don’t have an oil well, get one”. Well, my thought is, if you don’t have a dog, get one, and take him/her for a walk every day. Your heart will thank you, and your dog will faithfully follow you to the ends of the earth.

Dear Members and Friends of TOMA,

            Ah, just another perfect Sunday morning in North Dallas, with the temperature climbing into the 60s, clear blue skies, and the first election season is almost over. I don’t know about you, but I am no longer impressed by various claims of too much government, too much Washington, and too much cronyism in Austin. Politicians will do what they do, and we will pay the bill. Our national leaders called for ‘Change that we can believe in’. but the only change that I see is that they are going to pay for their programs with our income.

     The televised photo-op on health care finance reform occurred on Thursday, with the usual commentators wisdom supplied as if we cared. One of the statistics showed that the president talked for 1/3 of the session, with the other two stakeholders almost even. I guess since he called the meeting, and it was his home in which they were sharing their views, it was his right to answer every opinion with his own. Senator Tom Coburn of Oklahoma, a practicing physician, received the ultimate accolade when the president called him after the session for elaboration on his views. The senator called for tort reform and repeal of the SGR formula for CMS payments.

      The elephant in the room for all of us is the 21.7% cut in Medicare payments that will occur beginning on March 1st. It is my thought that Congress will have great difficulty in passing a permanent correction to the SGR formula because it is projected to cost around $280 billion over the next 10 years. The problem is that the Obama program has already claimed that money as savings for their universal health care reform proposal. The administration has said that their initiative would result in savings of $500 billion over ten years. That includes the money for physicians. We have always known that any ‘savings’ would cut benefits and access, and they can’t have it both ways.

Dear Members and Friends of TOMA,

            The TOMA 54th Mid-Winter Conference and Legislative Symposium was attended by a large group of your friends in Dallas. Program Chair Florence Spitler and committee chair Robert DeLuca coordinated a team effort that resulted in one of the best educational opportunities that we have ever experienced. The evaluations from both physicians and exhibitors were positive, and we were exposed to new treatment options and to contemporary ideas regarding older techniques. One of the new features was I-Click, an electronic interactive device that allowed presenters to ask questions that were answered by the attendees, and that computed outcomes data for each lecture. The weekend concluded with a comprehensive presentation by Texas Medical Board Executive Director Mari Robinson and Medical Director Alan Moore that summarized recent legislative changes that affect Texas physicians. The record snowfall in North Texas made travel difficult, but the convention facility was marvelous, and we had a great time connecting with our friends.

Dear Members and Friends of TOMA,

                'WHO DAT' ?  Congratulations to the New Orleans Saints and to all of their long-suffering fans. Sean Payton and Drew Brees brought their team back from a 10 point deficit to capture the NFL title. Queen Latifah started the festivities with a stirring rendition of America the Beautiful, and the team took it from there. Kenny Rodgers may be 'The Gambler', but coach Payton seized that name forever with his on-side kick-off to start the second half. Some of our TOMA folks will be in New Orleans for Mardi Gras that starts next week. This year may set a record for the longest celebration in history, as the revelry began last night.

 

                It is still winter in our lovely Texas, but the drought is officially over in many areas. The Northeast, including Washington D.C., was visited by a huge snowstorm this weekend, shutting down many government functions. Too bad it didn’t happen in the middle of the week, when closing the halls of Congress would mean that they couldn’t hurt us for a few days.

       One of the most dedicated educators at TCOM has left the public life for the rigors of private practice. Scott Stoll was honored by a reception in the atrium at the school for his contributions to the OMM department and to the Osteopathic Research Center. Scott has grown the department during his tenure and he has elevated the perception of osteopathic medicine on all levels, including research grants from the National Institute of Health. Interim department chair Russ Gamber joined other department faculty in paying homage to his courage, his vision, and his dedicated efforts on behalf of the students and the profession. He will be missed at the school, but he will be available for electro-diagnostic consultations at his new office in Fort Worth.

Presidential Newsletter - 1/25/10 Print E-mail

Dear Members and Friends of TOMA,

                To the surprise of no one in our Texas osteopathic family, the spirit of TCOM students has again risen to the occasion. When the news of the disaster in Haiti first broke, a small group of students began making plans to go to that devastated area to provide whatever assistance that they could bring. Former TCOM and current Rocky Vista Dean Bruce Dubin led TCOM students Joanna Gibbons, Bill Betz, Stephen Miller and others to a first landing in the Dominican Republic, followed by a trek across the border into Haiti. These dedicated volunteers, at their own expense, donated their medical skills and necessary provisions to the injured, and they helped lift the spirits of all that they encountered. Their stories will be recounted later in more detail, but I want to pay special tribute to these heroes of the profession who proved again that love of our fellow man knows no borders.

Dear Members and Friends of TOMA,

            I have rarely felt so inadequate as I have this week in watching the unfolding tragedy in Haiti. While this event is far from the usual interests of TOMA, as human beings and as physicians, we are immersed in the emotional upheavals that have befallen the millions of victims in that poor country. Through the miracle of modern communication, President Obama and other national leaders viewed the first pictures of the earthquake on Facebook. Volunteer physicians have flown to the island to help, only to find that the most basic supplies were not available. Water and food are beginning to be distributed, and U.S. and U.N. management is beginning to restore order. The masses of people have been described as quiet and stoic, and they will rebuild with our help. I urge you to choose your favorite charity and make a contribution that will assist in the international efforts to support the survival of these impoverished citizens of an already destitute country.

Dear Members and Friends of TOMA,

            Happy New Year, and welcome to the deep freeze. We are all proud of our Texas heritage, and we can usually brag about the mild Texas winter weather (except for Lubbock and Amarillo). However, the last week has been a reminder that Arctic air can find its’ way to our fair state, and we have all dug out our cold weather clothing. A low of 16 in Dallas made my dog run a little faster, and it made me wish to be back in Hawaii. Linda and I spent the holidays on a cruise to the much warmer islands, and instead of a snowy Christmas, we spent the day on Hilo at 80 degrees. We are glad to be back to work, and we are ready to resume our efforts for the profession.

The bad news is that we took the cold and drizzly North Texas weather with us. It was just plain miserable outside, but that was completely nullified by the warmth of the greetings from our members inside. TOMA executive director Sam and Gail Tessen and ATOMA president Mona Sanderlin also enjoyed the hospitality provided by Steve and Paula Gates and by Audrey Jones and Keith Hackland.

TOMA activities this week included an executive committee conference call on Tuesday night, a TCOM Task Force call on Wednesday night, a presidential visit to District 15 in Arlington on Thursday, an executive committee meeting in Austin on Friday evening, and a board meeting on Saturday. Somehow, I missed the instruction manual for slowing down for the holiday season.

Dear Members and Friends of TOMA,

             If you are reading these notes firsthand and not via our heavenly network, it means that you have enjoyed Thanksgiving, you have survived Black Friday (although the credit card bills are yet to come), and that you have safely navigated another weekend. Tiger Woods should be so lucky. It would seem that his Turkey Day ended on a discordant note when he had a disagreement with a fire hydrant over which club to hit out of the water. His wife may have helped him with the club selection, and she may have helped direct the swing. OMG, the problems associated with earning over $100 million per year. I will take the joys that come with my medical practice, I am very content with earning three zeros less than that, and I prefer the joys of working with my osteopathic friends in TOMA.

Dear Members and Friends of TOMA,

            This has been a truly momentous week for TOMA and for the osteopathic profession. On Friday morning, your humble servant was joined by AOA executive director John Crosby, AOA president Larry Wickless, Michigan State University College of Osteopathic Medicine Dean Bill Strampel, David Garza, Robert DeLuca, Jim Froelich, Scott Stoll and Joanna Gibbons in our effort to educate the UNT Board of Trustees regarding the risks of the allopathic degree proposal. The Academic Affairs committee of the board was chaired by Dr. Charles Mitchell, an orthopedic surgeon from Mesquite, who looked out upon a packed room of interested persons. Your representatives presented comprehensive information that included the historic relationship of TOMA and TCOM, the growth of the osteopathic profession in the U.S. and in Texas, the exorbitant costs of creating and maintaining an allopathic degree program, and the unintended consequences that could ensue from the massive commitment of UNT in underwriting this proposal. The Price-Waterhouse-Coopers report was summarized by their representative, and TCOM Provost Tom Yorio and president Scott Ransom delivered strong personal endorsements for the proposed degree.

     It just seems like the good news never stops, at least for UT, TCU, SMU, and Boone Pickens U. Our good friends from Tech, A&M, and Baylor are disappointed, and UNT continues to celebrate their world-class marching band. It is beginning to look like the Longhorns will play for the national championship against whoever survives the game between Florida and Alabama. My neighbor down the street has augmented his usual orange flag and his license that proclaims HORNS 75 with an array that displays his true devotion to the cause. Fort Worth is ablaze with Frog Fever for their magnificent team that has risen to BCS #4. Two Texas teams in the top 4 is a powerful statement about the power of ‘Friday Night Lights’, and the training gained in high school football in the Lone Star State.  

          Your team at TOMA has been busy during the last week, and next week will be the latest step in the efforts of your profession to educate the public concerning the folly of the M.D. proposal at UNT. Jay Beckwith and David Beyer arranged an interview between a leading faculty member at TCOM and Charles Gehren, a highly respected Texas state representative from Fort Worth. This dedicated faculty member shared information about the school executives who have spent almost of all of their time on this initiative for the last 18 months, information that only insiders would know. This assignment of work has taken them from their previous task of strengthening the Health Science Center by devoting considerable time and HSC salary money to promoting the M.D. program. Rep. Gehren firmly stated that he would not support any effort that weakened TCOM, and that he found great difficulty with the credibility of the leadership at the Health Science Center.

Dear Members and Friends of TOMA,

     Although it seems like yesterday, four months have passed since you gave me the honor of becoming your president. I have used this newsletter to let you know the activities of your TOMA, and you have assisted me with commentary about content and direction. During that time, several rather important issues have intervened to capture the attention of our profession, and these issues have provided me with fertile ground for commentary.

      First, this week, I want to bring you up to date on the U.S. Congress work on federal health finance reform legislation. This has been on the front page of every newspaper, every television talk show, and every browser home page for weeks, and most of us now understand that the program will probably include a requirement for universal health coverage, paid for by healthy 25year old U.S. citizens, and by reductions in payments for physicians from Medicare. This week, the House bill was endorsed by AARP and the AMA. It is quite easy to understand the involvement of AARP, as they are really a mail-order insurance company, and they will gain millions of additional policies with government subsidies. The support for this proposal by the AMA is a little harder to approve, and has been controversial in their own ranks. The AMA counts 23% of allopathic physicians as members, compared to 68% for the AOA. The bill contains almost none of the priorities of organized medicine, and includes only a weak promise to consider some type of tort reform. Here is a link to the summary of this bill’s contents.

Dear Members and Friends of TOMA,

            I know that TOMA has absolutely nothing to do with football, or any other sport. However, one does not live by bread alone, and many of our members are proud fans of their alma maters. After disappointing our Red Raider members, the Texas A&M Aggies have again demonstrated that old saying “on any given Saturday”, anyone can win.   UT and OU won as expected, and TCU has seized control of the Mountain West. SMU again struggled against a ranked team on the road, and the UNT marching band was the high point in Troy, Alabama. My Oklahoma State Cowboys picked the Bears clean in Waco, and now trail only Texas in the Big 12 South. Next week, the Longhorns venture north to Boone Pickens U., in the game that will decide the conference leadership. Official events at the AOA convention in New Orleans are going to be in conflict for those of us who are interested in the ball game.

Dear Members and Friends of TOMA,

            Longhorns Rule. The Red River Rivalry resulted in another UT victory for the nationally #2-3 ranked team. In a game marked with mistakes and an exceptional number of official reviews, the proud tradition continued with a win for Texas. UT quarterback Colt McCoy probably won the game not with a touchdown pass, but with a touchdown saving tackle on an interception. The Aggies, the Bears, and the UNT Mean Green had their struggles, but TCU, and Texas Tech won handily. At the TOMA district 5 meeting on Thursday, I was taken to task for not mentioning SMU. This week, the festivities at SMU were elevated on Saturday by OSU’s favorite son, Boone Pickens, who sponsored a star-studded promotion for the wild mustang project of his wife. The local Mustangs were defeated by Navy, but it took an overtime to do it.

            One of our own was recognized in the Dallas Morning News regarding his commendable contributions to the history of aviation. Former TOMA president Bill Hospers and former ATOMA president Chuckie Hospers have created a second career by establishing the Vintage Flight Museum at Meacham Airport in Fort Worth. The museum contains 20 rare and historic planes, and features a fully restored, working, B-17 bomber appropriately named Chuckie. This plane is one a very few existing examples that are still flying. Dancers jitterbugged to entertain surviving members of the 95th Bomb Group, which flew 334 B17 missions during World War II. This was the 19th annual Hanger Dance, held to raise funds to keep the planes in pristine condition. Bill’s skills as an orthopedic surgeon have been transformed into the mechanical expertise necessary to supervise the repairs and rehabilitation required to keep these birds flying in order to preserve their heritage of winning the war.

Dear Members and Friends of TOMA,

            Many are a little apprehensive of anything that is numbered 13. In the U.S., one will never find a 13th floor in the hotel or on a cruise ship. However, this is the 13th newsletter that I have penned, and I refuse to believe that it is unlucky. Aggie and Baylor Bear fans might think they would rather see the Okies return north, but Texas Longhorn and Texas Tech fans like my 13th week. This is Texas-OU week, and the bragging rights currently reside South of the river.

            Last Tuesday, Linda and I joined TOMA executive director Sam and Gail Tessen and ATOMA president Mona Sanderlin in Temple for an official visit to District 18. This district has provided our organization with dedicated leadership from district president Jeff Rettig, past TOMA president and AOA leader Dan Saylak and Amy, TOMA board member Sprague Taveau and Jean, and TOMA board member and Jefferson award winner George Smith and Jean. Sprague presented a comprehensive program on wound healing, and Sam, Mona and I gave our thoughts about current issues in our profession.

Dear Members and Friends of TOMA,

            There is no longer any doubt that fall has arrived in North Texas. The skies in Dallas look like Seattle, the Rangers are saying ‘Wait Until Next Year’, and the Aggies are looking for Divine Redemption or Gubernatorial intervention after the Southwest Classic (the Dallas headline said ‘Hog-Tied’). Radar (Steve Bander) and Dean Gafford are celebrating the Baylor wins, and OU fans are wondering how to cure Sam Bradford’s ailing shoulder. The Horned Frogs are wiping up their rivals, UT has stayed at #2 in the rankings, and UNT is wondering if their fabulously successful music programs can show the football team how to win.  The forecast for Corpus Christi for today is 93 and clear, and Steve Gates, I am looking forward to coming to the coast for your district meeting.

            TOMA has many under-recognized geniuses. For instance, one can find humorous writing talent in the rural splendor of Wylie. Last week, I related the great experience that Linda and I had when we attended the District 13 meeting. Steve Bander penned the following much more entertaining depiction of that evening’s events.

Dear Members and Friends of TOMA,

            There is something magical about fall in Texas. The ‘Hook-em’ cries are mixed with ‘Hoo-ahs’, the days are 95 and nights are 60, and the politicians are jockeying for power. Actually, all of those things always happen the whole year round-just pick your favorite place in Texas to observe these natural wonders, and you can see almost anything. With the advent of the football season in North Texas, it is fairly obvious that UNT has been more successful in educating osteopathic physicians that it has been in recruiting football players. Universities change coaches when a new direction is sought, and perhaps the same motivation was applied to the Health Science Center. The very successful educational model of TCOM is currently under attack by those in Fort Worth who insist on establishment of an allopathic medical school at the Health Science Center. Previous TCOM presidents have listened to these entreaties and concluded that staying the course by growing the size and strength of the osteopathic student body would be more beneficial to UNT, Fort Worth, the student body, and the taxpayers of Texas.

            UNT Health Science Center President Scott Ransom has communicated with us in a second letter that further explains his reasoning that a medical college would benefit the hospitals in Fort Worth. Click here to read his letter.

            Well, you did it again. The dry season in Texas has abated thanks to the combined prayers of osteopathic physicians. There may have been a few million others that participated in the entreaties to a higher power, but I am positive that the readers of this letter produced most of the improved conditions. It is almost time to complain about too much rain and the possibility of flooding.

Dear Members and Friends of TOMA,

            This has been a busy week for your humble servants at TOMA. On Wednesday, David Beyer and I were interviewed separately by Price Waterhouse Coopers, the consultants on the proposed allopathic medical school at the Health Science Center. Unfortunately, both meetings were lacking in interactive communication, and the meetings were primarily a presentation by the lead consultant, telling us what the report would say. At one point in the session that I attended, a leading Fort Worth businessman who is a member of the Board of Visitors at the school noted that we had already consumed 2/3rds of the allotted time, and that the consultant had completed only ½ of the published agenda. He asked if there were plans to ask us for our conclusions regarding the issue. The consultant essentially said that we would be out on time, and that she would proceed with the agenda.

Dear Members and Friends of TOMA,

            Well, it is now September, and we can think about football instead of only such minor issues as federal health system reform, and the current debacle at TCOM. I have to betray my bias and cheer that Oklahoma State won, and is ranked in the top ten, as two of our children graduated from Boone Pickens U. Texas looked good, and Radar is thrilled that Baylor also prevailed. Oklahoma had a ‘schoolin’, but they will be back. Tech produced a predictably high scoring game, and that team will also be in the hunt. Our Aggies can again be proud of a strong start to the season in their victory over the powerhouse from the West.

            It is hard to believe that it is almost September, and the evenings have finally cooled. As I drank coffee and read the paper on the porch this morning, it was 69 degrees in Dallas. It was very pleasant outside doing Sunday morning yard duties. The neighborhood was active, with people walking their dogs, washing their cars, and cleaning up the driveways. Life is good in Texas, even when the daytime temperature reaches 100 degrees.

Dear Members and Friends of TOMA,

 

            Well, not to anyone’s surprise, August is still hot and dry in Texas. Dallas got some much-needed rain in the last few days, but I know that much of the state is still parched and in triple digits. Anticipating positive outcomes in medicine is much like waiting for September to come-you know that it is going to happen, but you would like to speed the process.

            The fall board meeting of the TOMA board of trustees will be held at the school on September 12th. Reports have been requested from members of the TCOM administrative staff regarding current plans and programs at the school, the board will receive a report from David Garza and the Task Force, and the board will then devote considerable discussion to determining further action. I would very much appreciate your comments regarding this issue, so that they can be shared with the administration. The petition opposing an allopathic degree can be downloaded at our website. August is a fairly quiet time in politics, both national and state. There is a lot of information being formulated by congressional staffers and lobbyists who did not get August off, but the real action will only begin in September. Executive Director Sam Tessen has been very busy assembling and disseminating information regarding the issues that will affect the house of medicine. Remember to communicate with your federal senators and representatives and let them know your thoughts concerning these issues. If you need an e-mail address or telephone contact for the politicians, Patty Bell at the TOMA office will be glad to assist you.

Dear Members and Friends of TOMA

            I have always known that our members had unlimited-powers, if we would just focus our efforts in the right direction. Last week I asked you to pray for rain, and good rains came to many areas of Texas. Keep up the great work.

           Former Speaker of the U.S. House of Representatives Tip O’Neil famously said that all politics is local. While President Obama and the federal politicians have tried to distract us from the stimulus package and the housing market by picturing the debate on health care reform as an emergency, we should remember the speaker’s words regarding several issues of vital importance to Texas osteopathic physicians.

            Last Saturday evening, AOA trustee Mark Baker and AAOA past-president Rita Baker hosted their annual pool party for incoming TCOM students. Linda and I were out of town and were unable to attend, but I know that this great event provided a family venue for the students to get acquainted with each other and with practicing Fort Worth physicians. We appreciate the dedication of Mark and Rita to the growth and development of osteopathic students.

Last week, the executive committee of TOMA met by conference call and voted to oppose the current bill on health financing reform that is before the U.S. House of Representatives. Our official position states

MOTION:  The Texas Osteopathic Medical Association opposes HR 3200 in its current form;  however, TOMA endorses the concept of health care reform and pledges to work with the Obama Administration and the Congress to develop a comprehensive plan for health care reform that is affordable, protects the sanctity of the patient/doctor relationship, and provides quality health care for the people of the US.  Specifically, TOMA opposes certain inclusions in the bill and is concerned about certain issues that are not included in the bill such as tort reform. TOMA opposes certain segments that are currently included in the bill, and wants further action to include support for the medical home, increased support for primary care graduate medical education, and removal of the SGR formula for computing physician fees. We also sent letters to Senators Cornyn and Hutchison expressing our concerns regarding the House bill. The final legislation will take form in a conference committee after Labor Day, and I encourage each of you to communicate your feelings with your federal politicians.

Dear Members and Friends of TOMA


      It has again been a busy week. Last Thursday, TOMA executive director, Sam Tesson and I had two meetings in Austin on TOMA business. The first was with Tom Manley at TMF to present a grant request to cover the costs of our planned Osteopathic Medical Student Summit in October. The purpose of the OMS summit is to join all of the stakeholders in an effort to create new undergraduate and post-graduate opportunities for osteopathic students, especially in community settings. The second meeting was with Mary Frazier, the appointments director for Governor Perry, to discuss D.O. physician selection to state regulatory bodies, including the TMB. We have asked the governor to inform TOMA when there are physician openings on state committees...
